About 40 percent of the calcium in blood is bound to albumin, so when albumin is low the measured total calcium reads low even though the physiologically active ionized calcium may be normal. The albumin correction estimates what the total calcium would be if albumin were a normal 4.0 g/dL.
The correction is an approximation and performs poorly at very abnormal albumin levels. When precision matters, a directly measured ionized calcium is more reliable than any formula.
